2024/25 Mid Season Survey
The 2024/25 Upper York Admirals Mid Season Survey is now available!
Completion of the Mid-Season Survey is critically important to the UYMHA Executive Board, so we can continue to offer the most productive, positive development, and successful growth of all members within the Upper York
Minor Hockey Association.
The survey is comprised of 6 sections: "Mid Season Review", "Player Development", "Skills, Tactics & Strategies", "Communication & Team Relations", "Conditioning, Injuries, Prevention Management" and "Mid Season Evaluation".
This is a checkbox format survey with the opportunity to add additional comments in each section. Each survey takes approximately 3-10mins. All surveys are completely anonymous unless the participant wishes to share personal information in the comment sections.
Surveys will close on Saturday, February 1st at 11:59pm
