Travel permits must be completed and approved for all tournaments/exhibition games and submitted at least TWO weeks prior to the event, to allow for processing with the OMHA office. No fee for OMHA tournaments. For tournaments outside the OMHA (e.g. GTHL, Alliance, USA), a $20.00 fee applies, which MUST be paid following submitting this form.
