Police Vulnerable Sector Check
Upper York Minor Hockey Association requires all persons in a position of trust to successfully complete a Police Vulnerable Sector Check (PVCS)
This includes the Head Coach, Assistant Coaches, Managers, Trainers and Volunteers.
Each year after you have completed your PVSC, you will be asked to sign a Criminal Offense Declaration Form to ensure that your status has not changed.
The objective of a PVSC is to protect Hockey Canada, the OMHA, the UYMHA and most importantly our children from the potential threat that a "person in authority" might present; if that individual has a criminal record that gives cause for concern.

Here's the steps that you need to follow:
1. Contact the UYMHA Office Administrator by email:
[email protected]
requesting the authorization letter from the UYMHA for volunteers.
2. Take this letter to apply via in person or online application portal.
In Person Applications:
PVSC applications being completed in person can be done at York Regional Police Headquarters.

Monday, Tuesday and Friday: 8 a.m. to 4 p.m.
Wednesday, Thursday: 8 a.m. to 6 p.m. (phone support from 10 a.m. to 6 p.m.)
Saturday, Sunday: Closed
On-Line Applications:
PVSC being completed via an on-line application portal can be picked up at any of the York Region police facilities (when completing the on-line application, it will give you the list of facilities to select from).
3. COST for volunteers is $32.00 and can take upwards of two weeks for processing. If you are required to submit fingerprints, the addition fee will be waived by providing the UYMHA volunteer form.
4. Upon receipt of your PVSC, please submit a copy to the Office Administrator and your file will be updated. Your PVSC is good for 3 seasons.
